Nominations can only be received from members of the Minor League Football Hall of Fame. Individuals cannot submit nominations on their own behalf.
1] Enter name of person submitting the nomination.
2] Enter the email of the person submitting the nomination.
3] Enter the HOF Class (year) of the person submitting the nomination.
4] Enter the phone number, including the area code, of the person submitting the nomination.
5] Enter name of the HOF "Nominee".
6] Include a biography of the HOF Nominee that includes the following...
a) Total years involved in Minor League football.
b) All teams he/she was associated with as a player, coach, team owner, executive (team or league), game official, or journalist/broadcaster.
c) Significant contribution to the sport of football at the Minor League level (i.e., awards, and honors, win/loss record, advancement of players to the collegiate and/or professional levels, respectively, etc)
